Messages from/to internal users going to Outlook's Junk folder (Outlook 2013, Exchange 2013 on-premise/cloud)?

I have a strange issue in my environment where some random messages from/to internal users are going to a user's junk folder.

Environment: Exchange 2013 (on premise), Exchange Online (hybrid), Outlook 2013

User A (on premise exchange mailbox) sends message to User B (on cloud). User B is running Outlook 2013 in cached mode (since their mailbox is in the cloud for best performance). Message from User A goes to User B's Outlook junk folder and both the sender/recipient are within the same organization / email domain!

What is more interesting, this is 1 message that got filtered, other messages from User A to User B get delivered to Inbox.

What can i do to Exchange 2013 on premise and cloud so that messages within the organization never go to Junk without disabling the Junk Folder on the User B's cached mode Outlook.

I tried to set a rule on both on premise/cloud Exchange to apply a SCL of -1 for messages sent from/to "Inside the Organization" and I see in the message header a SCL of -1 but the message is still junked.

Why is this? I want to bypass filtering for internal user to user messaging, or internal automated reports that use on premise exchange server as a relay.
